Various certificates that serve as necessary written proof for certain procedures are available to be issued at city hall and some other city facilities.

 

Copy of Certificate of Residence (juminhyo no shomei)

This application form is used when one needs a copy of one's certificate of residence or certain information listed on said certificate.

Who can apply

  • A person who is registered as living in Miyakonojo or someone from the same household
  • A representative (a Power of Attorney is necessary)

What is necessary

Where to apply

  • Miyakonojo City Hall, Citizens' Affairs Div., Service Counter #2 (shiminka)
  • The Citizens' Lifestyle Div. at one of the city's branch offices (shimin seikatsuka)
  • One of the regional citizens' centers
  • Iwayoshi Community Center, Yokoichi Community Center
  • Library (Saturdays and Sundays only; excluding national holidays and days the library is closed)

Hours of reception

(Monday – Friday)  8:30 A.M. – 5:15 P.M.

The Citizens' Affairs Div. is open until 7:00 P.M. on Thursdays.

The above community centers are open from 9:00 A.M. – 5:00 P.M.

The library is open from 9:30 A.M. – 5:00 P.M. (excluding the lunch period from 12時$3分 P.M. – 1:00 P.M.)

300 yen

Note

  • The library, while able to issue copies of certificates of residence, cannot certify personal items (such as official postcards in need of verification) brought with you.
  • Pre-applications via fax, phone or email are not accepted.
  • There are cases when the organisation that you submit the Certificate of Residence to requires to have the family connection (zokugara) and one's permanent residence (honseki) on the Certificate. Please confirm with the organisation prior to applying at the City Hall.
  • Please contact the Citizens' Div. with any inquiries.

 

Issuance of Copy of Certificate of Residence Outside of One's Registered Municipality (juminhyo no koiki kofu)

People living outside of Miyakonojo can have the City of Miyakonojo issue their Certificate of Residence.

  • A person who is registered as living outside of Miyakonojo or someone from the same household
